Application items should be mailed to OIA - Kent State University, P.O. Box 5190, Kent, OH 44242-0001. You should send items to us as you complete them so we can begin your application file. Please telephone or email us for an application packet: telephone - 330/672-7980, or email sbaker@kent.edu. ► Program Application: must be accompanied by a $30.00 check or money order to cover the non-refundable application fee. ► Transcripts covering all college/university work must be submitted to OIA.► Transcript Release authorization must be signed so your grades at the end of the semester can be sent to you and to your home school registrar.► Letters of Recommendation: Two (2) from college/university faculty members who have had you as a student.► Agreement Release: must accompany the application.► Photographs: Three-they must be nearly identical to those required for your Passport and should have a light background. Unacceptable photos will be returned, and will delay your application process.► Parental Permission Form is required of students who are under the age of majority in their state of residence (18 years of age in Ohio).► Passport Number/Expiration Date and Italian Student Visa though not required with your application, must be on file with OIA before a participant can depart for Florence. |
